Phoenix in Maricopa County is the capital city of the state. Arizona death records are managed and issued by the Arizona Vital Records Office. If the event occurred more than thirty days ago, you may need to apply for the death certificate in person at the State office of Vital Records. According to a survey taken in 2005, Arizona ranked 7th in US for deaths in accidents. The vital records are not public records in Arizona and only the spouse, family members, attorneys representing family members or estates and private investigators can apply for the death certificate. The spouse or family members of the deceased person can receive Arizona death records, by providing the proof of relationship, birth certificate or marriage certificate that links the children or spouse to the deceased and proof of legal interest. The children applying for the death certificate must be at least eighteen years of age. The death certificate includes information, such as the name of the deceased person, age, sex, date of death, place of death and the names of the father and mother.
